This form is to request an appointment. We always strive to accommodate the appointment time requested. When the actual appointment is made, you will receive an appointment confirmation. Please check your messages for scheduling confirmation.

During normal business hours, schedule requests are typically processed within an hour of being received. Someone will then contact you to confirm the appoint.

If you are a new patient, please go to the "Forms" page on the Clay Primary website to complete your online registration form. We cannot keep scheduled appointments if we do not receive registration forms.
