Can Not Log In to Portal / Reset Acct

If you are unable to log in to your patient portal, please fill out and submit this form. Within 24 hours, You will receive an email with your USERNAME and a TEMP PASSWORD. Click on link and choose a password.

Portal Facts: -Pay close attention to your USERNAME given in the email you receive. This MAY NOT be your email address -Emailed link can only be used one time -Patients may change username or password from the acct setting tab in the portal -All documents/results from our office will be found under the MESSAGES tab -Each time an item is put in your portal, you will receive an EMAIL that states "You have a MESSAGE from your provider"
