Your reliance on the information provided in Up Dox is not a replacement for proper medical attention. The information displayed in UpDox may not be the complete medical record. Therefore, you must contact Dubuque Neurology & Sleep Medicine P.C. directly for official and complete copies of your medical record or in regard to discrepancies with the medical information listed in your record. Up Dox may not be available to you at all times due to system failures, procedures, maintenance, or other causes beyond the control of Dubuque Neurology & Sleep Medicine P.C.. Access is provided on an "as-is, as available" basis kind Dubuque Neurology & Sleep P.C. does no guarantee that you will be able to access Up Dox at any particular time. Dubuque Neurology & Sleep Medicine P.C. will make its best effort to provide a timely response to your electronic messages. In some situations, the clinic staff that needs to respond to a message may not be immediately available. Therefore, the anticipated turnaround time for a response to a patient inquiry is 2 business days. Your electronic messages may be shared with the staff at Dubuque Neurology & Sleep Medicine P.C. that assists your physician in providing your medical care. However, your messages will only be available to appropriate clinical staff. If our provider is out of the office or unavailable, messages sent within UpDox may be routed to other appropriate and authorized caregivers within our office in order to facilitate a timely response to your request. In addition, Dubuque Neurology & Sleep Medicine P.C. is only able to respond to electronic messages based on the information provided by you. Therefore, if there is insufficient information provided, Dubuque Neurology & Sleep Medicine P.C. will be unable to provide accurate and reliable services.
UpDox access is granted for the purpose of furthering your medical care. Use of UpDox unrelated to this purpose may result in the suspension or termination of access privileges. Improper use included, but is not limited to, the use of inappropriate, threatening or abusive language, requesting appointment times that are frequently cancelled, and any other use that Dubuque Neurology & Sleep Medicine P.C. determines in its sole discretion constitutes a disruption of its operations. Dubuque Neurology & Sleep Medicine P.C. reserves the right, in its sole discretion, to terminate a user's access to all or part of the UpDox website, with or without notice.
You will be provided with an access code when you initially enroll in UpDox. You must use the access code that is given to you to create a unique personal login name and password. Once the access code has been used, it will no longer be valid. The login name and password you choose will be used to access your health information in UpDox and are unique codes that identify you in the UpDox system. Any inquiries and entries you make in UpDox will be logged with your identity and may become part of your official medical record. Therefore, it is extremely important that you keep your login name and password completely confidential. Anyone with access to your login name and password will be able to access your health information, as well as read your messages and send new messages as if they were you. It is your responsibility to prevent disclosure of your login name and password, and to change your password if you feel that the security of your password has been compromised. You may change your password at any time. You may do so by logging into dnsm.myupdox.com, choosing "my profile and enter a new password then click update"
Your enrollment in Up Dox is contingent on verification of your identity in person by a Dubuque Neurology & Sleep Medicine P.C. employee
All communication between you and your Dubuque Neurology & Sleep Medicine P.C. provider occur over a secure connection. However, if you elect to receive internet e-mail messaging notifying you that new information is available in your UpDox account; please take the following into consideration. Although these e-mail messages will not contain you medical information, the notification that new information is available by accessing UpDox may be information that you do not want other to know. Therefore, you should take this into account when providing Dubuque Neurology & Sleep Medicine P.C, with your email address. All of the health information available to you is accessed within your UpDox account and is protected and securely maintained by Dubuque Neurology & Sleep Medicine P.C. staff. Although UpDox is configured to be secure from unauthorized access, Dubuque Neurology & Sleep Medicine P.C. is not responsible for: 1.Absolute security of all electronic communication transmissions between the patient and Dubuque Neurology & Sleep Medicine P.C. 2.
