    Welcome to the Tampa Heights Medical Center Patient Portal.

     This Patient Portal is your gateway to more convenience and control over your healthcare. For example, you can fill out forms right here instead on waiting to get to the office -  select the "Forms" tab at the top of this page, then select the appropriate form(s). Fill out selected form(s) correctly and completely before submitting. New patients, please fill all forms and send a copy of your driver's license, the front and back of your insurance card. This information is needed 24 hours prior to your appointment. It can be upload here through the portal. Click "login"; then Sign in; [top right corner] Click "send message"; Click "add attachments".
     Making appointments or checking in with the doctor about test results can all be done right here - safe, secure, and convenient.  Ensuring your health information is protected and your experience is easy and enjoyable.

     Another convenient feature we offer is Telemedicine, a live video conference through your phone or computer. Visit the TELEMEDICINE page for more information.

Please read carefully the new changes and reminders of existing policies.

1.  There will be a $50 fee for all forms. We want to make sure you get credit for your hard work in staying healthy so there is NO fee for GO365 biometric screening forms, Well4Life screening forms and Head Start communicable diseases forms.

2.  In order to make sure everyone?s issues are addressed in a timely fashion we ask that the patient portal is used to:

* complete new patient and annual forms,

* schedule or cancel an appointment,

* send documents to be added to your chart i.e. Blood pressure log, colonoscopy reports, MRI results, other medical records,

* and notify you of test results.

 Any other concerns should be called into the office during normal business hours.

3.  Telephone conversations to address a problem will be billed to your insurance provider, except notification of test results.

4.  Referral authorization requests need to be submitted two weeks in advance so we can make sure you are able to keep your scheduled appointment.


5.    We want to make sure you stay well and not running out of your medications is important. Please check your supply periodically and request refills at least 1 week in advance.
