If you are a NEW PILOT, appointments for FAA exams can still be requested using the "forms" button (above).
TO REQUEST A REMOTE CONSULTATION: IF NOT URGENT, PRESS THE "FORMS" BUTTON ABOVE, AND SELECT "TELE/VIDEO CONSULTATION REQUEST". IF URGENT, CALL THE OFFICE DURING USUAL HOURS, OR MY CELL AFTER HOURS.
FOR REGISTERED PATIENTS AND PILOTS:
SIGN IN using the button on the top right. (Username is usually the e-mail we have on file for you. This may be a previous/older/work e-mail, or an alternate username provided to you.) If you know your username but you have forgotten your password, click the button to sign in anyway (upper right), and you can request a new password to be sent to the e-mail we have on file for you currently. If you do not receive it, check your spam folder. If you are unsuccessful, see below.
IF YOU ARE UNABLE TO LOG IN:
Payments can stil be made safely on this page by using the "make a payment" button (above) if you cannot log in, but for better record keeping we advise you do so after logging in if possible. Enter the amount of your bill/payment, and provide a brief "reason" as a reference for the payment.
Referrals can be requested without logging in by using the "forms" button (above), but if you prefer secure communication we advise you do so after logging in.
Appointments for FAA exams can be requested using the "forms" button (above). (Note: If you are an established pilot and are able to log in, you can choose and book your exact appointment.)
Medical apppointments for established patients can also be requested using the "forms" button (above). (Note: If you are an established patient and are able to log in, you can choose and book your exact appointment.)
Refills can be requested on current medications using the "forms" button.
You may request a new username using the "forms" button (above) or by email at admin@drzwaan.com or drzwaan@gmail.com
AFTER YOU ARE SIGNED IN:
You can change your username and password using the "my profile" link (top right) if you wish. To review your demographics, contact information or insurance use the "patient information" and "patient profile" tabs. (Use "edit" and "update" only if you see errors or have changes please.)
To send a secure new message, use the "send message" link (top right). You can securely send me files as an attachment!
To review messages and reports sent to you, choose the "messages" button. (You may keep your messages on file for future reference as a personal health record, reply, forward, print, or export them in pdf form to use as you wish.)
You can request refills on current medications and referrals using the "forms" button.
You can make, review, confirm or cancel your appointments using the "appointments" button. (Call or email if you need a specific time that is not available.)
You can make payments using MC/Visa using the "make a payment" button (also see below.)
TO FIND US, DO NOT DEPEND ON GPS, AS THE EVEN AND ODD NUMBERS ON HAMPTON ROAD DO NOT MATCH UP. We are at 19 Hampton Road. Look for the green sign marked "EXETER EXECUTIVE PARK", diagonally across from Churchill's garden center, on the drive shared with Langdon Place assisted living.
