#5 Labs And Prescription Policy

Kenneth Stark, MD, PA

We require laboratory blood testing to assure your safety on the chronic medications you are taking. for YOUR protection and under no circumstances will we renew certain prescriptions if current copies of lab results are not within your chart. When you are given a prescription for lab testing, you must abide by how often to do the test and obtain the specific test we request. It is your responsibility to make sure you do the test in a timely manner and that we obtain a copy of the results. If you have labs done at another doctors office it is YOUR responsibility to make sure the right tests that we requested are done and the results are sent to us. If you miss an appointment and do not reschedule, your prescription will not be refilled. If you are running low on medications, call your PHARMACIST for the refill, they will verify by computer with us and the prescription will be renewed. Please give the pharmacist at least 3 DAYS notice. ONLY call us if the pharmacist tells you to do so, or if you change pharmacies. Again, we need 3 business days to process the request so do not wait until the last minute. By doing these simple things prescription renewals will be a seamless process.
