In order to schedule your vaccine appointment, you must create an account and complete the patient registration. 

We will then approve your patient account which will allow you access to schedule an appointment for your COVID-19 vaccine. 
**We will notify you when your account has been activated.
 
Before your appointment, please fill out our Immunization Consent Form located under the 'Forms' tab.
