TO LOG IN FOR THE FIRST TIME: Your default User ID is the email address we have on file for you. You should receive an email containing a temporary password. You will be prompted to change it to a password of your own immediately after entering the temporary password.
IF YOU ALREADY HAVE A PORTAL ACCOUNT, BUT HAVE FORGOTTEN YOUR PASSWORD: Please click on "Click Here to Sign in". Then click "Forgot Password". Type in your Username and you will receive an email with a temporary password.
IF YOU ALREADY HAVE A PORTAL ACCOUNT, BUT YOU DO NOT KNOW YOUR USERNAME: Click on FORMS and FORGOT MY USERNAME and submit the form. Someone from our office will contact you to assist you with getting logged in.
IF YOUR EMAIL ADDRESS CHANGES: Please notify us if you change your email address so that you will receive notifications when you have new items to view. Note that if you are using your email address as your Username, updating your email address with us will not change your Username. You will need to log into the portal and change it by clicking on My Profile.
PRESCRIPTION REFILLS: If you are in need of a prescription refill, please contact your pharmacy and they will contact our office.
MAKE A PAYMENT: You may make a payment on your account by signing into the portal and clicking on Forms and Make a Payment. (If you have forgotten your Username or Password, see the instructions above under "USERNAME and PASSWORD").
MAKE PAYMENT ARRANGEMENTS: You may set up a monthly payment plan by signing into the portal and clicking on Forms and Make Payment Arrangements.
