Dear Patient:

To begin, I would like to thank you for the trust you have given me over the years as your physician. Taking care of you has been an honor for my staff and me. Your continuing wellness and health have been a priority for us.

I have closed my practice effective October 02, 2020. I will not be available after that date.

It is important that you continue with appropriate medical care; therefore, you should establish with another physician as soon as possible.

To find another doctor, the following is provided:

Option I: If you are managed care situation, you may need to contact your healthcare insurer for a referral within in the network.

Option II: You can contact the Greater Albuquerque Medical Society at (505) 821-4583 to find doctors who are taking new patients.

All our records are stored in a secure Electronic Health Records system and will be released to your new physician upon receipt of a HIPPA compliant authorization form filled out and signed for release of information. On receipt of the signed form, we will forward a copy of your medical records to your new physician you designate.

After the close of this practice, request for copies of medical records may be directed to Fax number (505) 234-1707, or mail to Dr. Kevin Goodluck, P.O. Box 10460, Albuquerque, NM 87184.

If you have not selected a new physician by the time my practice closes, you can obtain acute, critical, or emergency care by calling 911 or go to any urgent cares or Emergency Departments in Albuquerque.
